My door sticker recommends one rear pressure for light load, and a higher pressure for max load. When filling my tires to tow, do I need to check the pressure with my 5th wheel hooked up or without, or does it really matter?

No, it doesn’t matter. The air pressure in your tires will not be affected by the load. If your hauling a heavy load for any long distance, it’s a good idea to check them regularly through the trip.

Every hour or so, stop and get out, put your hand on the tire and see it doesn’t get too hot.

Hot weather, asphalt roads and heavy loads are what causes blow-outs.

It doesn’t matter with the load when checking the pressure.
However, the temperature of the tire does.
Always check tire pressure when air in tires are cold ( before driving ), the pressure in the tire will go up in relation to the higher temperature of the tire, tire temperature increases with driving.
